Permit & Certificate Request Forms

This page contains all of the permit request forms required by the F.S.M. Division of Civil Aviation for air operations within the Federated States of Micronesia and also provides instructions regarding mandatory supplementary documentation.

FIXED-WING AIRCRAFT FLIGHT PERMIT REQUEST FORM

All fixed-wing aircraft with the intent to operate in the FSM and FSM airspace must apply for the following flight permit.

  • Requests must be submitted at least 24 HOURS IN ADVANCE.
  • This form must be accompanied by color scans of the pilot's license (front & back), medical certificate, passport information with photo, and last page of log book.
  • The aircraft Certificate of Registration and Airworthiness Certificate must also be submitted.

HELICOPTER FLIGHT PERMIT REQUEST FORM

All helicopters with the intent to operate in the FSM and FSM airspace must apply for the following flight permit and must provide complete information as to the manner of transportation.

  • Requests must be submitted at least 24 HOURS IN ADVANCE.
  • Manner of transportation examples include, but are not limited to: (1) a helicopter arriving or departing in a container, (2) a helicopter being transferred from vessel to trailer via crane, or (3) a helicopter being transferred from anchorage site to airport via flight.
  • This form must be accompanied by color scans of the pilot's license (front & back), medical certificate, passport information with photo, and last page of log book.
  • The aircraft Certificate of Registration and Airworthiness Certificate must also be submitted.

FIXED-WING AIRCRAFT LANDING PERMIT REQUEST FORM

All aircraft with the intent to land within the FSM must apply for the following landing permit.

  • Requests must be submitted at least 7 DAYS PRIOR TO ETA.
  • This form must be accompanied by color scans of the pilot's license (front & back), medical certificate , passport information with photo, and last page of log book.
  • The aircraft Certificate of Registration, Airworthiness Certificate, and Air Operator Certificate must also be submitted.

MILITARY LANDING PERMIT REQUEST FORM

All military aircraft with the intent to land within the FSM must apply for the following landing permit.

  • Requests must be submitted at least 7 DAYS PRIOR TO ETA .
  • This form must be accompanied by a diplomatic note.

OVER-FLIGHT PERMIT REQUEST FORM

Any aircraft passing through FSM airspace must apply for an over-flight permit.

  • Requests must be submitted at least 48 HOURS PRIOR TO ENTERING FSM AIRSPACE .

FOREIGN AIR CARRIER PERMIT REQUEST FORM

Any foreign air carrier with intent to operate in FSM airspace must apply for the following permit.

  • This form must be accompanied by copies of the Foreign Investment Permit, Insurance Policy, Company Operations Manual, Company Security Program, Maintenance Control Manual, Air Operator Certificate, Certificate of Public Convenience & Necessity, Aircraft Registration & State of Registry, and Certificate of Airworthiness for each aircraft to be used.

AIR OPERATOR CERTIFICATE REQUEST FORM

Any air operator with intent to operate in FSM airspace must apply for the following permit.

  • This form must be accompanied by copies of the Foreign Investment Permit, Insurance Policy, Company Operations Manual, Company Security Program, Maintenance Control Manual, Aircraft Registration & State of Registry, and Certificate of Airworthiness for each aircraft to be used.

Instructions & Policies

  • (1) Permit request forms will not be processed unless they are complete and accompanied by all required flight documentation.
  • (2) Color scans of flight documents are preferred.
  • (3) Permits will not be issued unless request forms are submitted in a timely fashion in accordance with the instructions. Time needed for the review of a request differs from form to form. Ample advance notice is always recommended.
  • (4) Permit requests, their attachments, and/or questions should be submitted via electronic mail to ncaa@mail.fm.
